These four tenets are found in the Foursquare Healthcare logo and the company name (Foursquare). The tulip is the traditional “get-well” flower and the “four squares” represent our foundational services – nursing, therapy, pharmacare and hospitality. Your Rehab Team

MANY DISCIPLINES — ONE GOAL


Once the patient is on the road to recovery, the rehab therapy team rolls into action. The team consists of physical therapists (PT), physical therapy assistants (PTA), occupational therapists (OT), certified occupational therapy assistants (COTA), a speech and language pathologist (SLP), and a rehab aide/technician, who work together based on the individual needs of each patient.

The Physical Therapist assesses and treats impairments in strength and function that occur as a result of injury, disease, age-related degeneration, or environmental factors. Physical therapy treatments are performed by the physical therapist, or a physical therapy assistant who treats under the direction of the supervising physical therapist.

The Occupational Therapist assesses the impact that a decline in function has in the patients’ ability to participate in their life by performing everyday tasks such as dressing and bathing, or being able to return to work or engage in previous hobbies. Occupational therapy treatments are performed by the OT (or a COTA working under the direction of the supervising OT).

The Speech Therapist and/or Language Pathologist assesses language, cognition, and swallowing dysfunction.  A Speech Therapist’s main goal is helping resident with swallowing difficulties.  They will work with residents that may have modified diets to help them get back to eating the types of foods they love.

These three disciplines work together along with nursing, social services, activities, the business office, families, caregivers, and local communities to assess functional potential, and collaborate on the unique goals for every patient. Family members are essential contributors to this interdisciplinary rehab team, as they provide the background and detail of their loved one’s life to incorporate into our plan of care.

While in our care, please know that your loved one will be well taken care of. Because all of our rehab therapists are in-house (rather than outside contractors), our relationships with our residents are superior. Our therapists and nurses work together as a team, and due to the continuity of their care, a strong trust is built with our residents. There is comfort in knowing that the same smiling face will be there to help them through their rehab each day.

Your Care Plans

For the Family:  Tailored So YOU Can Make a Difference!

Shortly after admission to our facility, the Interdisciplinary Care Team (IDCT) will meet with you and your family members to develop a Care Plan.  Each discipline (Nursing, Therapy, Nutrition, Activities, Social Service) of the IDCT asks questions to make sure you receive the unique services your individual situation requires.  When a patient isn’t able to effectively communicate, family members are heavily relied upon for key information needed to help the patient feel comfortable and get as well as possible.  A broad spectrum of information about the patient is taken into consideration, from medical data such as diagnosis and prior level of function, to personal information such as likes and dislikes, religious preference, social and family support and personal history and interests.

Getting Started: The Care Plan Meeting

Every effort is made to schedule the Care Plan Meeting to accommodate the availability of the patient and family, to ensure that the patient and family participate in the development of the Care Plan to the fullest extent possible. Family members and/or the responsible party are encouraged to attend the meeting. The Care Plan Meeting is typically conducted within the 1st or 2nd week after admission.

The Care Plan Meeting involves the Interdisciplinary Care Team (IDCT) which includes, but is not limited to, the following professionals:

• Nursing Representative

• Dietary Services Supervisor/ Registered Dietician

• Social Services Representative

• Activity Director

• Treating Therapist or Representative

• The Administrator, Director of Nursing, Attending Physician, Nursing Assistant responsible for resident, and others may attend as needed

As an interested family member, this is your opportunity to get your questions and concerns answered, and we’re here to help. We understand what an anxious process this can be, so we take our time explaining the details of what your loved one’s care and stay with us will involve, how you can assist in their recovery, and what to expect along the way.

Once a plan is in place, frequent updates are given to involved parties to improve the plan of care and ease any concerns.

The Discharge Plan: Get Well and Go Home

Discharge planning is initiated upon admission as the Care Plan is developed by the IDCT. Included in the discharge planning is the resident’s discharge outcome goal. The Discharge Plan provides a “road map” for returning the patient to their home and prior level of function, or to the highest level of function possible.

The discharge outcome goal is a reflection of the professional opinion of the team based on the initial evaluation, and is open for change based on the patient’s progress. The discharge outcome goal includes the following components:

• It will be developed with the resident and/or family goals in mind.

• It will include destination of the resident, sources of assistance available to the resident, level of assistance needed by the resident, adaptive equipment needed, and appropriate referral required.

The Outpatient Care Plan: Home Or Away, We’re Still Here to Help

Our outpatient programs are designed so that our patients can return to our therapy clinic for continued therapy if needed. The same therapists they got to know during their stay with us can continue to follow up with the patient through outpatient therapy. Our therapy programs are also often involved in local Assisted Living Centers and Senior Centers to provide wellness classes, ongoing training and education, and health screenings.

WHAT IF I’M NOT HAPPY WITH THE CARE I’M RECEIVING?

Our Care Confidant Program is designed to promote communication between you and staff. A staff person will be assigned to you and will check your room each day to make sure everything is clean, orderly and there are no housekeeping concerns. This person is also there to listen to what’s on your mind and if possible, resolve any concerns. This is an extra layer of care that we provide to all our guests to ensure their stay is a positive experience.

WHEN WILL I RECEIVE THERAPY AND WHAT SHOULD I WEAR?

We offer physical, occupational and speech therapy throughout the week. You will be evaluated as soon as possible upon arrival and a personalized therapy plan will be designed to best meet your individual needs. Comfortable loose-fitting clothing and tennis shoes are recommended.

HOW DO I RECEIVE MY MEDICATIONS?

Your medications can only be ordered once you have arrived and your information entered into our system. We should have your medications 4-6 hours of your arrival. In the event you need pain control, please request pain medication to be administered 30 minutes-1 hour prior to your departure from the hospital. We ask that you do not keep any medications in your room or self-medicate to avoid serious medical complications.

WHEN WILL A WALKER BE PLACED IN MY ROOM FOR ME TO USE?

Until your therapist determines that you are safe to use a walker by yourself, we encourage you to use the wheelchair provided in your room to reduce risk of falls.

WHEN WILL I RECEIVE MY SHOWER?

Showers are three times a week on a rotating schedule. Please consult your nurse aid for your personal shower schedule.

WHAT IS A 5-STAR RATING FROM MEDICARE?

Nursing home ratings come from: health inspections, staffing, quality measures. A star rating is provided for each of these 3 sources. Then, these 3 ratings are combined to calculate an overall rating. Learn more

WHAT SHOULD WE BRING FOR A SHORT TERM REHAB STAY?

Prepare an inventory list of items you’re bringing with you

Copy of medicare/medicaid or insurance card

Medical records

Copies of living will and power of attorney

3-4 days worth of comfortable clothes (there are washing facilities on location), night clothes and toiletry items

Something from home that might bring you comfort like a favorite quilt, photos, etc.

HOW LONG WILL I BE HERE?

Length of stay is based upon your individual needs. Factors include your current physical condition, underlying medical conditions, previous levels of independence and your discharge location.

DO YOU CONTACT MY PRIMARY CARE PHYSICIAN (PCP)?

Upon arrival we will let them know that their patient is at our facility and after your discharge, your PCP will receive a copy of your records from your stay with us.

HOW DO I TAKE CARE OF MY LAUNDRY?

The facility provides complimentary laundry services or you may arrange for your family to do your laundry. Please clearly mark your name on all your clothing items.

It goes without saying that the change is going to be biggest for the person actually making the move. If the transition is into independent living, your parent’s probably eager to play an active role in selecting a new home. It can be a fun, exciting process—and we’ll help you both make the most of it. If your parent isn’t up to playing a big role, we’ll help you evaluate the other options at Juliette Fowler and choose the perfect fit.

Understand your resources.

The cost of senior living varies a great deal. Identifying the funds you have to work with will help you and your parent focus on options you can afford—before you fall in love with a community that’s outside your budget. What are your parent’s current monthly income and expenses? Are the sources of income stable? Are there upcoming expenses that need to be factored in? Will selling a home or other property help cover ongoing living costs? You’ll need all of this information and more when you apply to a community, so start gathering it early.

WHAT IS AN INTERDISCIPLINARY REHABILITATION PLAN?

A plan designed by an interdisciplinary team of health care professionals from diverse fields who work in a coordinated fashion toward a common goal for the patient. An intraprofessional team is a team of professionals who are all from the same profession, such as three physical therapists collaborating on the same case.

WHAT IS A MODALITY IN PHYSICAL THERAPY?

It is used to relieve pain, improve circulation, decrease swelling, reduce muscle spasm, and deliver medication in conjunction with other procedures. Typical types of modalities include electrical muscle stimulation, ultrasound, heat, ice, and traction.

WHAT IS SHORT WAVE DIATHERMY?

Diathermy is a therapeutic treatment most commonly prescribed for joint conditions such as rheumatoid arthritis and osteoarthritis. In diathermy, a high-frequency electric current is delivered via shortwave, microwave, or ultrasound to generate deep heat in body tissues.

WHAT IS INTRAVENOUS THERAPY?

IV therapy is the infusion of liquid substances directly into a vein. Therapies administered intravenously are often included in the designation of specialty drugs.

WHAT IS POST ACUTE CARE?

Post-acute care settings include long-term care hospitals (LTCHs), inpatient rehabilitation facilities (IRFs), skilled nursing facilities (SNFs) and home health agencies.

WHAT ARE THE DIFFERENT THERAPIES OFFERED?

Physical therapy provides evaluation, examination, and treatment of neurological disorders, musculoskeletal conditions, cardiac patients, wounds, and other diagnosis.  Physical therapy also provides recommendations for discharge placement assessment for the needs of assistive devices (durable equipment), and functional mobility.  The wound care team is led by a certified wound care specialist (CWS).

Occupational therapy provides evaluation, examination, and treatment for activities of daily living (ADLs), adaptive equipment and durable medical equipment training, neuromuscular re-education, splint fabrication, sensory deficit compression techniques, cognitive retraining associated with ADLs and safety, and energy conservation techniques.

Speech therapy provides evaluation, diagnoses, and treatment of communicative, cognitive, and linguistic disorders.  Modified barium swallow studies and bedside swallow evaluations are used to objectively assess swallowing function.
